By using our services, you confirm that you are at least 18 years old or have parental consent to engage in our offerings. Our services are intended to promote well-being but do not replace professional medical, psychological, or emergency care.
3). Appointments and Cancellations
Appointments can be booked or cancelled online or in person.
Cancellations must be at least 24 hours in advance to avoid cancellation fees.
Late arrivals may result in a shortened session without a fee reduction
4) Payments and Refund Policy
Payment is due at the time of booking unless otherwise specified.
We accept cash, credit/debt cards, carecredit, checks, and other approved payment methods.
All sales on services and retail items are final, except where required by law.
Package and membership purchases are non-refundable but may be transferable with approval.
5). Health and Wellness Disclaimer
Our services are not a substitute for professional medical or psychological care.
You acknowledge that participation in our programs is voluntary and at your own risk.
Please consult a licensed healthcare provider before beginning any new wellness or therapeutic practice.
Dr. Cheryl Graham is a Doctor of Chiropractic (DC), is not a medical doctor (MD) and doesn’t diagnose, treat, or cure any disease. Reading the LionHeart Chiropractic & Functional Medicine website does not mean that you are a patient of Dr. Cheryl Graham, does not mean you are a coaching client with Dr. Graham or any other associates of LionHeart Chiropractic & Functional Medicine.
All individuals need to remember to consult your medical doctor when making any changes to your diet, prescription, medications, lifestyle or exercise activities, and to go to urgent care, ER, or call 911 if experiencing a health crisis, emergency, or life-threatening illness.
The intent of any training, classes, coaching, webinars whether live or recorded is purely education and designed to follow the functional medicine & nutrition principles of supporting the natural physiological and biochemical processes of the human body.

Follow all LionHeart Chiropractic & Functional Medicine guidelines and policies.
Refrain from disruptive or inappropriate behavior.

Your privacy is important to us. We collect and store personal information only to provide services and improve your experience. We do not sell or share your data with third- party except as required by law. See separate Privacy Policy section for more details.
HIPPA & Privacy Statement: Our practice is committed to protecting your personal health information(PHI). We comply with HIPAA regulations regarding the confidentiality of your health records. Any information shared with us is used only for treatment, payment, or healthcare operations, and will not be shared with unauthorized third parties. 8) Limitation of Liability
LionHeart Chiropractic & Functional Medicine is not liable for any injuries, damage, or losses incurred while using our services or premises.
We are not responsible for lost, stolen, or damaged personal items.
Our liability for any claim is limited to the amount paid for the service in question.
9) Changes to Terms & Conditions
LionHeart Chiropractic & Functional Medicine reserves the right to update these terms at any time. Changes will be posted on our website, and continued use of our services constitutes acceptance of the revised terms.
